Job Summary

We are looking for a CNAs and PCAs to join our team! You will directly work with clients, following a one-on-one service plan in client's homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets clients needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Private Home Care or Home Health Care experience is a plus.

Responsibilities

Assist clients with daily activities such as moving in and out of beds, baths, dressings, grooming, meal prepping, vitals, shopping, and wheelchair, or Hoyer lift assistance

Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care needs

Maintain records of daily tracking of client care, condition, progress, or problems to report and discuss observations with the supervisor

Build relationships with clients by conversing or reading aloud to help keep them mentally healthy and alert

Give medication reminders

Transporting, Scheduling and/or accompanying clients to medical appointments

Engage patients in exercises or other activities

Qualifications

Current TB Skin Test required

Driver’s license required

BLS CPR certification required
