Job Opportunity: Physical Therapist

We are looking for a dedicated and empathetic Physical Therapist to become a vital member of our Healthcare & Medical Services team. In this essential clinical role within our rehabilitation program, you will deliver evidence-based physical therapy interventions aimed at helping patients regain functionality, alleviate pain, enhance mobility, and achieve greater independence across various care environments.

• Conduct thorough patient evaluations, including in-depth assessments of musculoskeletal, neuromuscular, and functional capabilities.
• Design and execute personalized care plans that comply with physician directives, organizational standards, and the latest best practices.
• Utilize therapeutic exercises, manual therapy techniques, gait and balance training, modalities, and patient education to support patients throughout their rehabilitation process.
• Work collaboratively with a multidisciplinary team, which may include physicians, nurses, occupational and speech therapists, social workers, and case managers, to ensure patient-centered care.
• Maintain accurate documentation of assessments, treatment sessions, progress notes, and discharge summaries in the electronic medical record, ensuring clarity and compliance with regulations.
• Provide clear guidance and education to patients, families, and caregivers regarding home exercise programs, safety measures, mobility techniques, and equipment usage to facilitate smooth transitions across care settings and promote long-term functional improvements.
• Monitor patient progress, reassess functional status, and modify treatment plans as necessary to achieve optimal outcomes.
• Engage in quality improvement initiatives and adhere to all relevant state and federal regulations, as well as organizational policies concerning documentation, infection control, safety, and ethical practices.
• Assist in the orientation and mentoring of new team members, therapy assistants, and students, fostering a collaborative and educational environment.
