Wound Ostomy Nurse (RN) - Infection Prevention (Days)

Advanced Wound Care Nurse

To serve as the clinical expert to facilitate assessment, planning and intervention of patients who have or are at risk for the development of pressure ulcers or ostomies. The Advanced Wound Care Nurse will coordinate the development of best practice through a multidisciplinary team approach, continually evaluating performance and coordinating process improvement to ensure optimal patient outcomes.

Education Bachelor's Degree *Required

Experience One year of related experience. Requires a working knowledge of standard practices and procedures.

Licenses & Certifications *Georgia Registered Nurse License or Multistate Nursing License *Healthcare Provider (CPR) Qualifications *Certified Wound, Ostomy, and Continence Nurse (CWOCN) required *One year wound care experience or comparable experience type of care.

Area of Responsibilities

  • CLINICAL PRACTICE GUIDELINES AND STANDARD OF CARE: Facilitates multidisciplinary implementation of evidence-based practice for wound and ostomy management and prevention, through clinical protocols, assessment and prevention strategies. Serves as a resource for the assessment and plan of care for patients with complex wounds, or of patients with high risk for development of hospital acquired pressure ulcers.
  • OUTREACH: Coordination of program outreach with the goal of strengthening relationships with community partners, in order to improve communication and continuity of care across the continuum. Development of patient-family support.
  • PERFORMANCE AND OUTCOMES: The Wound-Ostomy nurse will be responsible for program evaluation and institutional performance through the development of performance measures with nationally recognized benchmarks. The Wound-Ostomy nurse will facilitate review of performance and development of action plans to ensure progression to top decile performance for each facility.
  • PROGRAM DEVELOPMENT: Works collaboratively across the patient Services Continuum to develop an Institutional Wound Prevention and Management program with effectively utilizes resources, standardizes care and optimizes patient outcomes.
  • STAFF AND RESOURCE DEVELOPMENT: Coordinate Institutional Education to facilitate consistent application of Standards of Practice and risk reduction strategies.

Compliance Statement Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

Contact With Others Appreciable contacts as regular part of the job with others outside of the department or organization. Requires discretion and tact to give or get specialized information to perform duties of job.

Effect Of Error Probable errors may be serious and involve losses such as improper costs, overpayment, waste of material, damage to equipment, and delay in processing work. Effect usually confined within the organization. Most of work not subject to direct verification or check. Regularly works with some confidential data such as account, salaries, patient medical records, which if disclosed might have adverse internal or external effects.

Supervisory Responsibility Exercises no supervision, work direction, or instruction of other employees or students.

Mental Demands Work involves a variety of complex problems to be solved under general organization policies. Ingenuity and judgment are required to review facts, plan work, estimate costs, and deal with factors not easily evaluated, interpret results, draw conclusions, and take or recommend action. Solutions to problems often require coordination with other departments.

Physical Effort Moderate physical effort - Lifts, carries, or handles lightweight (1 to 25 lbs.) materials or equipment for about half of the day. Very occasional physical effort with medium weight objects (25- 60 lbs.). Office or laboratory work requires close visual effort and concentration more than half of day. Works in reaching or strained positions for less than half of day.

Working Conditions Considerable - (About 75% of the day) Involved in exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes/occupational risk and probability of coming into contact with blood borne pathogens, other potentially infectious diseases, or biomedical/bio-hazardous materials.

Occasionally (at least once a month) Reaching - above shoulder Lifting 25 To 60 lbs. Squatting Smelling N95 Respirator usage (PPE)
