We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic team. As an LPN at Bradford Manor Nursing and Rehabilitation, you will play a crucial role in providing high-quality skilled nursing and rehabilitation services to our residents. About the Role:
As an LPN, you will be responsible for providing direct patient care, administering medications, and monitoring vital signs. You will also be expected to document patient records accurately and assist with care planning. You will be working closely with our interdisciplinary team to ensure the delivery of comprehensive care to our residents.
Responsibilities:
Provide direct patient care, including vital signs monitoring, wound care, and transfers
Administer medications and injections as ordered by physicians
Document patient records accurately, including medical histories and treatment plans
Assist with care planning and implementing care strategies
Collaborate with the healthcare team to develop and implement effective patient care plans
Maintain a safe and clean environment for patients and staff
Participate in quality improvement initiatives and maintain compliance with regulatory requirements
Requirements:
Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania
High school diploma or equivalent required
Ability to lift, transfer, and reposition patients as needed
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment
