Position: Case Manager, Supported Housing Full-Time/Part-Time: Full-Time Location: Medford, NY Salary: $55,000 Days/Hours: Monday - Friday; 9am - 5pm Description: Concern Housing is seeking a dedicated Case Manager responsible for providing direct case management services to clients. The role involves being a resource familiar with community resources and services to assist clients in gaining access, as well as advocating for their needs. Responsibilities include but are not limited to: Providing case management services such as service planning, medication monitoring, and assistance with clinical services, day programs, vocational services, community integration, and entitlements. Offering individual counseling and support services; responsible for documentation. Assisting with housing services, including maintenance issues. Conducting site visits as needed and directed by supervisor. Performing monthly fire safety inspections. Resolving landlord and lease issues, including lease negotiations and billing. Attending weekly supervision and staff meetings. Providing crisis intervention, including 24/7 cell phone coverage. Remaining current with all agency compliance documents and trainings. Position Requirements: Bachelor's Degree in Psychology, Rehabilitation, Social Work, or related mental health field, with at least 2 years of experience working in similar settings or with individuals with mental illnesses. Good verbal and written communication skills and sound judgment regarding client care are essential.
