About the Role

The Care Coordinator is the heart of our client care operations - ensuring each client receives consistent, high-quality care tailored to their needs and lifestyle. Youll work closely with families, caregivers, and community partners to create seamless, well-supported care plans. This role blends organization, heart, and leadership to keep our clients safe, supported, and comfortable in their homes.

Key Responsibilities

• Serve as the primary point of contact for new client referrals and care transitions
• Coordinate personalized in-home care plans with compassion and attention to detail
• Ensure continuity and consistency of care by matching clients with the right caregivers
• Monitor client satisfaction and well-being through regular check-ins and visits
• Support families with guidance, resources, and care planning insight
• Supervise and support caregivers, including performance evaluations and ongoing communication
• Collaborate with healthcare professionals, insurers, and internal teams to streamline services
• Participate in rotating on-call responsibilities to maintain service continuity

Qualifications

• Education : High school diploma or GED required; college degree or relevant certification preferred
• Experience :
  • At least 2 years in a healthcare or home care setting
  • Experience in care coordination or client services strongly preferred
• Licensure (must have one of the following valid in Georgia):
  • Licensed Practical Nurse (LPN)
  • Certified Nurse Aide (CNA)
  • Personal Care Assistant (PCA)

Training & Skills Required

• Solid understanding of elder care, chronic illness, disability support, and home-based safety
• Strong communication and relationship-building skills
• Competency in meal support, hygiene assistance, infection control, and home safety procedures
• Ability to document services accurately and follow care instructions
• Must meet state-required training standards
• Must pass background, reference, and driving record checks
• Valid drivers license, reliable vehicle, and current auto insurance
