Registered Nurse - Hospice

$2422.50/week -

Job Summary:

Provides comprehensive hospice nursing care to patients in their homes, ensuring high-quality, patient-centered services. Manages clinical assessments, care planning, medication reconciliation, and interdisciplinary communication to optimize patient comfort and safety.

Responsibilities:

• 
  Perform thorough patient assessments to determine appropriateness for hospice care, learning needs, and goal progression.
  
  
• 
  Develop and evaluate individualized care plans based on evidence-based practices and patient needs.
  
  
• 
  Ensure accurate documentation and timely transfer of assessment data, including admissions, re-visit notes, and discharge summaries.
  
  
• 
  Educate patients and families using age-appropriate methods, and revise teaching plans as necessary.
  
  
• 
  Supervise home health aides, coordinate scheduling, and communicate patient progress during interdisciplinary team meetings.
  
  
• 
  Maintain a safe environment by adhering to infection control protocols, managing medication safely, and reporting infections.
  
  

Qualifications:

• 
  Must have a valid New Jersey RN license and at least two years of RN experience in a home hospice setting.
  
  
• 
  Demonstrate proficiency with EMR systems, critical thinking, and strong communication skills.
  
  
• 
  Possess a valid New Jersey drivers license and BLS certification.
  
  
• 
  Show knowledge of hospice regulations, insurance reimbursement, and pharmacology.
  
  
• 
  Ability to prioritize workload and collaborate effectively within interdisciplinary teams.
