Position Highlights

\n

\n
• Full-time, day shift schedule
\n
• One weekend per month
\n
• Two holidays per year
\n
• Limited on-call responsibilities
\n
• Care for patients throughout the community
\n
• Autonomy to manage your daily schedule while supported by an interdisciplinary team
\n

\n

Primary Responsibilities

\n

\n
• Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes
\n
• Perform comprehensive patient assessments and ongoing evaluations
\n
• Develop, implement, and update individualized care plans
\n
• Administer medications, treatments, and IV therapy as ordered
\n
• Educate patients and caregivers regarding disease management, medications, safety, and wellness
\n
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals
\n
• Facilitate discharge planning and transitions of care
\n
• Maintain accurate and timely clinical documentation
\n

Qualifications

\n

\n
• Current RN licensure in Delaware or eligibility through a Compact State License
\n
• BSN preferred; ADN candidates may be considered with a commitment to obtaining a BSN within the designated timeframe
\n
• Minimum of one year of RN experience
\n
• Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred
\n
• IV therapy and wound care experience preferred or willingness to complete training
\n
• Current valid driver's license and reliable transportation
\n

Hourly Pay Range: $36.10 - $57.76\n

\n

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.\n
