The OpportunityFuntoNetwork is seeking a Nursing Home Psychosocial Services Consultant for upcoming project opportunity: Licensed Clinical Social Worker Services .
Core Responsibilities- Assess resident needs relevant to Wayne County nursing-home social work.
- Discuss Wayne County nursing-home social work concerns with residents and their families.
- Identify barriers affecting residents receiving Wayne County nursing-home social work.
- Prepare individual support goals for Wayne County nursing-home social work.
- Provide the assigned Wayne County nursing-home social work counseling and psychosocial interventions.
- Arrange referrals and care-team input needed for Wayne County nursing-home social work.
- Support families participating in Wayne County nursing-home social work.
- Document the Wayne County nursing-home social work assessments and interventions.
- Review changes in residents receiving Wayne County nursing-home social work and adjust support plans.
- Contribute staff education based on recurring Wayne County nursing-home social work support needs.
- A current resume or professional profile showing experience directly relevant to Nursing Home Psychosocial Services Consultant - Wayne County Nursing Home and Rehab Center - Licensed Clinical Social Worker Services.
- Examples of comparable work, outcomes, deliverables, and the tools or methods you personally used.
