Field-Based RN Case Manager Wound Care/Infusion

Adaptive Home Health is seeking a skilled RN Case Manager to join our field-based team in Texas. You will own patient assessments, document OASIS, and deliver high-acuity care with emphasis on wound care and infusion therapy, while coordinating with physicians and families.

You’ll work in a supportive system with technology that reduces documentation burden, a clear productivity model (points per week), and mission-driven goals to expand access to home health care.
