Overview Dialysis Clinic, Inc. is recruiting top talent to support our nonprofit mission of prioritizing individualized care for patients facing chronic kidney disease. The Dialysis Social Worker assesses and responds to the psychosocial needs of dialysis patients to ensure the best possible outcomes and improve quality of life. Schedule: 32 hours/week, M-F 8:00 am‑4:30 pm, with Saturday administrative rotation. Compensation: $72,800 – $98,000 annually, depending on dialysis experience. Responsibilities Provide patient support addressing emotional, financial, and lifestyle changes linked to chronic kidney disease. Assist patients in accessing community resources for nutrition, emotional support, transportation, and housing. Advocate for patients in accessing health care and other benefits. Educate patients and families on end‑stage renal disease treatment options including dialysis, transplant, and medical management. Coordinate translation services for patients and caregivers. Assess family support systems and refer for additional services as needed. Provide counseling to patients and families to help them cope with kidney failure, follow treatment plans, and achieve rehabilitation goals. Develop and implement individualized care plans with the interdisciplinary team based on psychosocial assessment. Collaborate with the interdisciplinary team to assess ongoing needs and modify care plans. Educate interdisciplinary team members on emotional aspects of dialysis. Advocate for patients at the clinic, community, and in state/federal policy settings. Serve as ESRD Network liaison. Engage in data‑driven quality improvement processes to positively impact outcomes. Qualifications Excellent communication and active listening skills. Desire to collaborate with care teams. Ability to problem‑solve and adapt. Education & Training Master’s Degree in Social Work (MSW) required. Licensed in the state of practice as required by state regulations. New MSW graduates welcome; job shadow opportunities available.
