Serve as medical liaison for IDT team, ensuring proper medical follow up. Develop written plan for each person and modify as necessary. Maintain medical records for each individual. Participate as appropriate in planning and implementation of training of Brightway personnel. Accompany individuals to medical appointments as assigned. Ensure all medications are administered and stored as necessary. Attend monthly nursing meetings, as required. Complete appropriate incident reporting paperwork, as required. Ensure individuals receive proper medical care. Ensure compliance with OPWDD standards. Act as liaison with family members regarding medical issues. Prepare monthly Medication Administration Records. 24-hour on call as necessary. (Residential only) What You Bring To The Role N.Y. State Registered Nurse License required N.Y.
