Physical Therapist - Travel Contract

Minimum Requirements

• Associate, Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Florida Physical Therapy license required prior to the start of the assignment.
• Home health care experience is required. Experience providing PT services in patients' homes, including documentation of services provided.
• Familiarity with home health travel routines and traveling between patient locations within a shift.
• BLS certification from AHA or ARC may be required.
• Preferred: HCHB (Homecare Homebase) EMR experience.

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Shift hours: 8:00 AM to 5:00 PM.
• Work days: Monday through Friday.
• Weekly guaranteed hours: 32 to 40.
• Productivity expectation: 30 points.
• Coverage area: Indian River County, St. Lucie County, and Martin County. Travel between patient homes is required. Mileage between visits will be reimbursed.
• On-call/weekend requirement: None.
• Required interview format: Teams.
• Dress code: Scrubs or slacks; any scrub color acceptable.
