Required experience is two (2) years in a home health setting within the last ten (10) years. Nurses must have recent experience conducting comprehensive assessments of patients. All nurse assessors must meet the following requirements. Experience may include working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Nurses should have provided primary care to homebound persons. Care management for individuals receiving home and community-based services is also relevant. Preferred qualifications include experience conducting assessments in UAS-NY, especially in the last 365 days. Experience can include caring for a geriatric population and managing chronic diseases in the home setting. Home care experience as a paraprofessional or LPN caring for a minimum of two patients per week is beneficial. Home care experience involving only one patient or telephonic assessments does not satisfy the required qualifications. Candidates must indicate which class they are submitting for on their resume. All candidates must be under the impression they will be conducting hybrid tele and in-field assessments. Job responsibilities include performing initial in-home evaluations of patients. Nurses will evaluate health status, strengths, care needs, and preferences. They will guide the development of individualized long-term care service plans. Reviewing consumer medical documentation and health referral forms is essential. Entering evaluation data into the electronic evaluation form and transmitting it is required. Documentation of any concerns or issues surfaced during the evaluation process is necessary. Emphasis on continuity of care is important to reduce fragmentation and gaps in treatment plans. Discussion with consumers on healthcare options, supports needed, and service vendor options is part of the role. Initiating communication with eligible consumers for enrollment into managed care is needed. Enhancing communication and relationships with interdisciplinary care team members is critical. Collecting quality review data and required documentation to support outcome measurements is needed. Nurses must identify opportunities for health promotion and illness prevention. Maintaining knowledge of community resources and payor requirements for the target population is essential. Meeting established standards for this position as outlined in performance criteria is expected. Other duties may be assigned by management as required.
