Snowline Health is seeking an experienced, compassionate Social Services Director to lead our social services programs and support our interdisciplinary hospice care team. This role ensures delivery of high-quality psychosocial care that aligns with patient goals, promotes dignity, and supports families throughout the care continuum. Responsibilities include: Provide leadership and oversight of social services programs and staff Supervise, mentor, and support social workers and related team members Ensure delivery of compassionate, high-quality psychosocial care Lead program development and continuous improvement initiatives Collaborate with interdisciplinary leaders to optimize patient outcomes Ensure compliance with regulatory and accreditation standards Oversee documentation quality, productivity, and workflow efficiency Support staff engagement, training, and professional development Partner with community organizations and external agencies Assist with complex patient, family, and staff situations Promote a culture of compassion, accountability, and excellence Salary Description Competitive, market-aligned salary based on experience and internal equity Requirements Master of Social Work (MSW) required Licensed Clinical Social Worker (LCSW) required Advanced certification (e.g., ACHP-SW) preferred Minimum of 5 years of clinical experience in healthcare, hospice, or related field Minimum of 3 years of leadership or supervisory experience Experience with program development preferred Experience supporting patients and families with grief, loss, and end-of-life care Strong leadership, communication, and interpersonal skills Ability to work effectively with diverse populations #J-18808-Ljbffr Snowline Hospice of El Dorado
