Form Health, a virtual obesity medicine clinic, is seeking an Eligibility Specialist to help patients determine program eligibility, collect records, and guide them through the process. This full-time remote role reports to the Manager, Eligibility and can be located anywhere in the USA. You will review patient records for eligibility, request and track medical records, and communicate clearly with patients and clinicians to keep them informed and compliant. #J-18808-Ljbffr Form Health
