• Contract Weeks: 13
• Guaranteed Hours: 40

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness for care.
• Document and prepare/review clinical record documentation, including Plan of Care, progress notes, and phone orders.
• Plan and provide appropriate treatment for patients with neuromuscular, neurological, and orthopedic disorders.
• Instruct patients and families on transfer techniques and the use of rehabilitation equipment.
• Educate patients/families on therapeutic procedures tailored to their age, condition, and learning readiness.
• Oversee and assist team members in performing patient care consistent with individual patient needs.
• Apply concepts of infection control and universal precautions in patient care activities.

Compliance / Notes:

• Must meet mandatory continuing education requirements.
• Ability to handle heavy lifting, bending, pulling, pushing, and standing for prolonged periods.
