Geriatrician (MD/DO)
Position Summary
The Geriatrician provides comprehensive, patient-centered medical care to older adults, with a focus on the prevention, diagnosis, and management of complex medical, functional, cognitive, and psychosocial conditions. This role emphasizes interdisciplinary collaboration, care coordination, and optimization of health, function, and quality of life for aging patients.
Essential Duties and Responsibilities
- Provide comprehensive primary and consultative care to older adult patients
- Evaluate and manage chronic medical conditions, multimorbidity, and geriatric syndromes such as frailty, falls, dementia, delirium, incontinence, and polypharmacy
- Conduct comprehensive geriatric assessments addressing medical, functional, cognitive, emotional, and social needs
- Develop and implement individualized, patient-centered care plans
- Provide preventive care, health maintenance, and advance care planning
- Coordinate care transitions across inpatient, outpatient, post-acute, long-term care, and community settings
- Collaborate with interdisciplinary teams including nursing, social work, therapy services, pharmacy, and case management
- Educate patients and families regarding diagnoses, care plans, prognosis, and goals of care
- Participate in palliative care and end-of-life discussions as appropriate
- Supervise and educate residents, fellows, advanced practice providers, or trainees as applicable
- Maintain accurate and timely clinical documentation
- Participate in quality improvement, patient safety, and outcomes initiatives
- Comply with all regulatory, accreditation, and professional standards
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
- Completion of an accredited residency program in Internal Medicine or Family Medicine
- Completion of an accredited Geriatric Medicine fellowship
- Board-eligible or board-certified in Geriatric Medicine
- Ability to obtain and maintain unrestricted state medical licensure
- DEA registration or eligibility

Work Environment
- Outpatient clinics, hospital inpatient units, long-term care facilities, skilled nursing facilities, and assisted living settings
- Schedule may include clinic hours, inpatient consults, and call coverage depending on practice model
Physical Requirements
- Ability to sit, stand, and move throughout clinical environments
- Manual dexterity for examinations and clinical care
- Visual and auditory acuity sufficient for patient assessment and communication
