Registered Nurse (RN) - Case Management

Director Case Management

Schedule: Full-time Days (No Weekends)

Requirements:

• Bachelor's degree required
• Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required
• 3+ years of experience in overall acute care hospital case management required
• 2+ years of experience in case management leadership preferred

Roles & Responsibilities:

• Direct and evaluate departmental operations, including case management model and staffing.
• Ensure adherence to care coordination and discharge planning processes.
• Assess and improve department performance through data analysis and implementing performance improvement activities.
• Establish working relationships with key stakeholders, including CMO, CNO, and ancillary service leaders.
• Manage departmental resources to achieve productivity and quality goals.
• Oversee compliance with policies and Standard Operating Procedures (SOP).
