Role Overview

A healthcare facility in Muskegon, Michigan is seeking an experienced Physical Therapist to support patient rehabilitation and mobility services within a clinical care environment.

This onsite contract opportunity focuses on evaluating patient conditions, developing individualized treatment plans, and delivering high-quality physical therapy interventions while collaborating closely with patients, families, and interdisciplinary care teams.

The ideal candidate has strong clinical therapy skills, prior Skilled Nursing Facility (SNF) experience, and the ability to provide compassionate, patient-centered care across a variety of rehabilitation needs.

- Monday–Friday

- 8:00 AM–5:00 PM

40 Hours Per Week

Some Holidays Required

One Weekend Per Month Required

Key Responsibilities

Patient Evaluation & Care Planning

Perform initial and ongoing assessments of patient conditions

Establish individualized treatment plans based on patient needs and clinical findings

Revise and evaluate treatment plans as patient conditions change

Collaborate with patients and families regarding rehabilitation goals and care expectations

Therapy Treatment & Rehabilitation

Deliver physical therapy interventions using standard therapy techniques and clinical skills

Support patient recovery, mobility, strength, balance, and functional improvement

Monitor patient progress and adjust treatment approaches as appropriate

Provide patient education regarding exercises, recovery expectations, and rehabilitation goals

Team Collaboration & Supervision

Collaborate with interdisciplinary healthcare teams to coordinate patient care

Oversee activities of physical therapy assistants, students, and support personnel

Support positive patient outcomes through team-based rehabilitation planning

Maintain strong communication with healthcare staff and patient families

Documentation & Compliance

Maintain accurate patient documentation and therapy records

Follow facility policies, therapy standards, and healthcare regulations

Ensure compliance with clinical quality and patient safety requirements

Support efficient and organized therapy department operations

Requirements

Minimum 2 years of Physical Therapist experience required

Skilled Nursing Facility (SNF) experience required

Strong patient assessment and rehabilitation planning abilities

Experience developing and modifying treatment plans

Ability to work collaboratively within multidisciplinary care teams

Strong communication and patient education skills

Comfortable supervising therapy assistants and support staff
