Case Manager
Cardinal Hill Rehabilitation Hospital
Cardinal Hill Rehabilitation Hospital seeks a Case Manager to coordinate comprehensive discharge planning and services for patients recovering from stroke, brain injury, spinal cord injury, and other complex conditions. In this onsite role, you’ll collaborate with an interdisciplinary team to assess needs, develop individualized care plans, and secure post-acute services and resources. You will educate patients and families, manage insurance and authorizations, support safe transitions of care, and track outcomes in a mission-driven, patient-centered environment focused on clinical excellence and recovery of function and independence.
