We are seeking a compassionate and experienced Primary Care Physician Assistant to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Nashua, NH.

Collegial practice with solid APP support

W2, employed position with Foundation Medical Partners

Not-for-Profit, 501(c) Organization

40 Hours 50% direct patient care work and 50% coordinating and communicating patient care

Clinical Affiliate of Massachusetts General Hospital

Hybrid Work Schedule Potential remote options once fully trained

Qualifications:

Certified Physician Assistant

2+ years of Primary Care experience

Strong clinical skills, including assessment, diagnosis, and treatment planning.

Excellent communication, collaboration, and interpersonal skills.

Proficiency with EPIC EMR preferred

Ability to work independently and as part of a team.

Work Shift:

Mon-Friday 8-5, (With potential for hybrid remote work)
