divh2Nurse Navigator, OB/GYN/h2pThe Clinical Nurse Navigator/Coordinator functions as a member of the multidisciplinary team who has clinical nursing expertise in an assigned area. They focus on the clinical and educational aspects of care, acting as a clinical resource, advocate, and educator. They are present in the clinic and serve as a vital member of the care team./ppEssential Duties and Responsibilities/pulliDemonstrate a strong commitment to patient-centered care within an interdisciplinary environment./liliFollow up with patients on testing and procedure results in coordination with providers./liliContact referring physicians or imaging groups and contacting patients on any issues or items needed for visit./liliAssists with coordination of follow-up visits, orders, and referrals./liliInitiates/Participates in quality improvement, patient safety, and patient experience initiatives./liliParticipates in training and education of support staff./liliAssess factors of the patients past and present physical, psychosocial, and cultural baseline, health problems, and external support systems that may impact his/her ability to cope with the current health situation./liliProvides excellent customer service by demonstrating ICARE values and positive behaviors towards patients, families/visitors, providers, other department staff, and co-workers./liliCommunicates problems and concerns to supervisors and provides input to support total quality improvement activities/li/ulpClinical Responsibilities (based on assignment):/pulliUtilize advanced nursing knowledge and the nursing process to:/liulliAssess a patients physical, psychosocial, and cultural background./liliIdentify relevant health problems and external support systems./liliDefine a nursing problem list informed by medical diagnosis and assessment./liliSet realistic, measurable goals related to each problem./liliDevelop a care plan targeting resolution and prevention of nursing issues./liliEvaluate and document patient progress./liliCollaborate with healthcare professionals to implement a comprehensive care plan./li/ulliManage OB/GYN Triage Schedule/liliDeliver patient education tailored to individual healthcare needs (i.e. Diabetes Education, Gynecological Conditions, Birth Control)/liliAssist physicians in routine patient care:/liulliAssist with procedures and treatments, as needed/liliProvide patient education as needed./li/ulliMaintain clear and accurate nursing documentation and document all patient interactions in EPIC/liliAdminister medications./liliRecognize adverse signs and symptoms quickly and react in emergency situations./liliServe as a telephone/myChart triage nurse as assigned/liulliAssess patient needs and prioritize calls./liliProvide advice or direct in-office support./liliAssess walk-in patients and coordinate care as needed./liliRedirect calls appropriately and work collaboratively with support staff./li/ulliPap Tracking and Mammogram Tracking as assigned/liulliEnsuring that PAP information is up to date in the EMR/liliCoordinate with the provider regarding care plan/liliParticipate in PAP/dysplasia tracking process, follow up with lab results, to include phone and written contact with patients/li/ulliPostpartum Follow-Up as assigned/liulliReview delivery report and scheduling/liliFollow up patients that have recently delivered and ensure they have appointments scheduled/liliFollow up with hypertension patients and patients at high risk for maternal mood disorder to ensure proper follow-up/li/ulliFetal Monitoring as assigned/liulliManage the daily NST schedule/liliReview the US schedule to ensure that patients are scheduled appropriately/li/ulliMaintain professional development:/liulliEarn continuing education credits./liliSet professional growth goals with nursing leadership./liliParticipate in departmental meetings./li/ul/ulpOther Duties:/pulliPlan, implement, and evaluate nursing care./liliIdentify and correct deviations from department policies/procedures./liliEvaluate patient needs and clinical requirements./liliServe as a role model in professionalism and clinical conduct./liliSupport precepting and procedural training under the nurse managers direction./liliMaintain accountability for the quality of care provided and tasks delegated./liliUphold all ethical and legal standards in accordance with MFA guidelines./liliIncorporate evidence-based research into nursing practice./li/ulpMinimum Qualifications/pulliEducation: An associates degree or higher in Nursing, or an equivalent combination of training and experience./liliBasic fetal monitoring certification or ability to obtain in first 30 days/liliAHA-Basic Life Support/liliExperience in Family Planning preferred/li/ulpLicensure:/pulliA current and valid D.C. license as a Registered Nurse or evidence of application for licensure in D.C./li/ulpExperience:/pulliA minimum of 3 years of experience as a Registered Nurse, OB/GYN experience preferred./liliExperience in Family Planning preferred/li/ulpPhysical Requirements:/pulliPhysical Sit, walk, reach, bend, or twist for long periods of time in a clinical setting./liliMust be able to lift, carry, push, or pull up to 50 lbs. as part of the role./liliRegularly exposed to healthcare settings that may require personal protective equipment./liliRequires manual dexterity to operate a
