Position Status Full Time - Day Shift Position Location Hampton, VA at Careplex Hospital Responsibilities Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care. Certification / Licensure Licensed RN, LPN, RT or BSW required. Proof of education as LPN, RN, RT or BSW required. Experience 1 year of home care, discharge planning or case management experience required.
