Work Environment

As a Nurse Practitioner at Cityscape Healthcare Center, you will work onsite in a home care environment, allowing you to forge close relationships with patients and their families while providing care in the comfort of their own homes. Your typical work schedule will be from Monday to Friday, encompassing a total of 40 hours per week. This role requires you to be adaptable and efficient as you navigate various home settings to deliver personalized care.

Key Responsibilities

• Conduct thorough physical examinations and gather detailed medical histories from patients during home visits.
• Diagnose and manage both acute and chronic health conditions effectively, focusing on pain management protocols.
• Design individualized treatment plans in collaboration with a multidisciplinary team and modify them based on patient progress and feedback.
• Order and interpret various diagnostic tests, including blood work and imaging, to aid in the evaluation of patient conditions.
• Provide education to patients regarding disease prevention, management strategies, and lifestyle modifications to enhance their quality of life.
• Work closely together with physicians, nursing staff, and other healthcare professionals to ensure coordinated and optimal care for patients.
• Maintain comprehensive and accurate medical records in compliance with regulations to ensure continuity of care and effective follow-up.

Patient Population / Client Focus

Your work as a Nurse Practitioner will largely focus on patients dealing with chronic pain and related conditions. These individuals may face challenges affecting their mobility, daily activities, and overall quality of life. As a key member of the healthcare team, you will play an instrumental role in empowering these patients through education, support, and effective management strategies tailored to their unique needs.

Qualifications

• A minimum of 2 years of experience in a clinical setting, preferably in pain management or a related field.
• Current licensure as a Registered Nurse in New York State.
• Certification as a Nurse Practitioner from an accredited program.
• Strong clinical skills along with effective interpersonal and communication abilities to establish rapport with patients and their families.
• The ability to function independently while also contributing effectively to a collaborative healthcare team.
• Current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).

Preferred Experience

While 2 years of experience is a prerequisite, we highly value candidates with additional experience in pain management, home care, or geriatric nursing. Familiarity with electronic health record systems and comfort with telehealth practices is an asset.

Compensation and Benefits

This position offers an attractive salary range of $150,000 to $160,000 per year, commensurate with experience, in addition to a comprehensive benefits package. Benefits include:
