Advantage Care Health Centers - Responsibilities Provide patient-centered, team-based care within the PCMH model, coordinating with medical staff to address clinical, social, and behavioral health needs. Conduct intake evaluations and develop treatment plans. Provide individual therapy for children, adolescents, and adults. Maintain accurate documentation. Support warm handoffs, continuity of care, and collaboration with families, caregivers, and community partners. Support quality improvement initiatives, facilitate referrals, advocate for patients, and participate in interdisciplinary treatment planning.
