Qualifications & Skills Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care Unrestricted licensure in the state or ability to obtain it Reliable transportation and a valid driver’s license Ability to work in varied environments and perform physical duties as needed Preferred: 1+ years clinical experience in geriatrics, home health, or related fields; familiarity with EMRs and complex patient needs Key Duties Conduct comprehensive in-home assessments and screenings Manage acute and chronic conditions, including testing and interventions Document medical information thoroughly and accurately Engage in care planning, counseling, and patient education Collaborate with the care team to ensure holistic patient support #J-18808-Ljbffr Live Well Homecare
