Home Health Admissions RN
Job Summary:
Deer Meadows Home Health is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Home Health Admissions RN. As a vital member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes. Your expertise will enable us to deliver exceptional outcomes and improve the overall health and well-being of our patients.
Home Health Admissions RN Responsibilities:
Conduct initial assessments and develop comprehensive care plans for patients in their homes
Implement and monitor treatment plans, ensuring continuity of care and communication with patients, families, and healthcare providers
Collaborate with physicians, therapists, and other healthcare professionals to develop and update patient care plans
Educate patients, families, and caregivers on disease management, medication regimens, and rehabilitation strategies
Identify and address patient safety concerns, adhering to evidence-based practices and guidelines
Document patient progress and care activities accurately and timely, according to agency policies and regulations
Maintain a positive and respectful attitude, promoting a supportive and inclusive environment for patients, families, and colleagues
Home Health Admissions RN Requirements:
Current licensure as a Registered Nurse in the state of Pennsylvania
Prior experience required; experience in home health or a similar setting required
Experience completing Oasis assessments
Strong communication, interpersonal, and problem-solving skills
Ability to work independently and as part of a team
Willingness to learn and adapt to new technologies and procedures
