Loudoun Home Care is seeking a compassionate and dependable Registered Nurse to join our growing team. The RN will provide high-quality skilled nursing care to clients in their homes while supporting their safety, comfort, and independence.

Key Responsibilities

• Complete nursing assessments and develop individualized plans of care.
• Provide skilled nursing services, including medications, treatments, and wound care.
• Monitor clients’ conditions and promptly report significant changes.
• Maintain accurate and timely clinical documentation.
• Coordinate care with physicians, families, caregivers, and other healthcare professionals.
• Supervise and support LPNs, home health aides, and other assigned staff.
• Conduct required supervisory and reassessment visits.
• Educate clients and families about medications, treatments, safety, and ongoing care needs.
• Follow infection-control, safety, and quality-care standards.
• Participate in staff training and quality-improvement activities.
• Follow Virginia regulations, nursing standards, and agency policies.

Qualifications

• Active Virginia or Compact RN license.
• At least one year of nursing experience.
• Current CPR/BLS certification.
• Experience caring for clients with a tracheostomy and G-tube.
• Home care, community health, or private duty nursing experience is preferred.
• Strong assessment, clinical judgment, communication, and documentation skills.
• Ability to work independently and confidently in a client’s home.
• Compassionate, dependable, and professional approach to care.
