Job Summary:
Long Term Care (LTC) Nursing professional working in a post-acute care facility specializing in long-term and skilled residents. The role involves providing comprehensive nursing care for a diverse patient population, including residents with chronic conditions, post-surgical needs, and rehabilitative goals. The position emphasizes collaborative interdisciplinary teamwork in a facility dedicated to patient-centered care.

Job Details:

• Schedule: 12-hour days, with scheduled weekends as needed, and a minimum of 36 hours per week.
• Required Qualifications: Valid BLS Certification; minimum of 2 years of nursing experience in a comparable setting; appropriate state licensure for licensed practical nurses or registered nurses; located within a feasible distance to the facility.
• Preferred Qualifications: Experience with long-term residents, familiarity with Point Click Care electronic medical record system, and proficiency in wound care, medication administration, and patient assessments.
• Float: Float rotation may be required.
• Additional Information: Position involves passing medications, wound care, Foley and catheter care, PEG tube feedings, ostomy and trache care, and managing IVs including IV antibiotics, blood products, and IV fluids. Nurses will coordinate with an interdisciplinary team that includes PT, OT, SLP, social workers, case managers, and physicians, with available after-hours physician support. Orientation includes two shifts. No private COVID units, with enhanced droplet precautions and N95 respirator use.
• Charting System: EPIC.
• Collaboration: Attendance at team conferences may be required for certain resident care planning.
• Dress Code: RN - navy blue; LPN - navy blue; CNA - gray scrubs with any print top.
• Additional Notes: The facility maintains a COVID-19 vaccination policy; no trauma level designation or specific on-site pharmacy details are provided.
