Nurse Practitioner - Certified Wound Care (Clinic-based)

Nurse PractitionerThis position is a clinic-based Nurse Practitioner role providing comprehensive wound, incision, and ostomy care to patients in outpatient clinical settings. The Nurse Practitioner will provide care at various clinic locations throughout the assigned geographic area and will travel between locations as needed throughout the week. The Nurse Practitioner will evaluate and manage patients with acute and chronic wounds, develop individualized treatment plans, perform appropriate wound care procedures, monitor healing progress, and provide patient and caregiver education. The Nurse Practitioner will collaborate with physicians, nurses, medical assistants, referral sources, home health providers, and other members of the healthcare team to coordinate care and support continuity of treatment. The role requires strong clinical assessment and decision-making skills, effective communication, organization, and accurate and timely documentation. The Nurse Practitioner will primarily provide care in outpatient clinic settings. Limited virtual/telehealth or community-based care may be required based on patient, program, or operational needs.Education: Master's DegreeMaster's degree in Nursing from an accredited nursing program. Licensed as a Registered Nurse by the State Board of Nursing (SBON) in the state(s) where the nurse practices. Board certified and licensed as a Nurse Practitioner in the state(s) where the Nurse Practitioner practices.Specialty Education/Certification:Preferred: Completion of a WOCN Society-accredited educational program and current certification by the WOCNCB in wound care, including one of the following:CWCN – Certified Wound Care NurseCWON – Certified Wound Ostomy NurseCWOCN – Certified Wound Ostomy Continence NurseAlternative:WCC – Wound Care Certified through the National Alliance of Wound Care and OstomyCWS – Certified Wound Specialist through the American Board of Wound ManagementMinimum Experience:Nurse Practitioner: Minimum of one (1) year of experience in a wound-related Nurse Practitioner role with one of the followingThree (3) years of experience as a CWCN, CWON, or CWOCNFive (5) years of experience as a WCC or CWS
