Joint and Spine Nurse Navigator

Job ID 89108-147 Date posted 06/24/2026 Nurse Navigator – Baylor Scott & White Surgical Hospital Fort Worth The Nurse Navigator is responsible for providing patient guidance throughout the continuum of care for Total Joint and Spine service line patients. This professional will serve as the education lead for patients preparing for surgery and coordinate care for patients and their families. Responsibilities Assist in the development of education for patients and staff, aligning with performance measures for Total Hip & Knee Replacement and Advanced Spine Surgery certifications. Round on total joint and spine patients post‑operatively to anticipate discharge needs. Interact and facilitate care delivered by all care providers (nursing, physicians, physical therapists, pharmacy, radiology, case management, social work, etc.). Facilitate patient and family education by developing appropriate education programs, resources, and tools, including comprehensive preparation class schedules. Evaluate, develop, and update patient education material for accuracy and appropriate literacy level every two years or as needed. Demonstrate the ability to gain the patient’s understanding and acceptance of the Total Joint and Spine processes through patient feedback and data collection. Follow up with patients (rounding and phone calls) and providers in a timely manner to discuss patient response to treatment and obtain progress reports in conjunction with Centers of Excellence programs. Education Graduate of an accredited school of nursing. Licensure & Certification Current Texas RN License Current BLS Experience Minimum five (5) years’ experience as an RN in a hospital Medical/Surgical or ICU Unit.
