Community Health Navigator - Native Outreach & Transportation

The Community Health Representative (CHR) acts as liaison between medical provider and client to bridge gaps between Osage/Native clients and health resources. The CHR provides transportation to Osage/Native clients within the Osage Reservation who have no other means of transportation with priority to Wah-Zha-Zhe Health Center appointments and their referred appointments. The CHR strives to improve overall health and wellness and promotes disease and injury prevention of our Osage/Native community members.
