Nurse Practitioner

NP/PA PM&R Clinician working exclusively at a designated skilled nursing facility (SNF) or post‑acute rehab center. Location Palo Alto, CA and Los Altos, CA. 40‑hour, full‑time W‑2 role. Compensation Starting at $150,000.00 per year, with bonus pay. Schedule 8‑hour shift with flexibility; preferred hours 9AM–4PM, Monday–Friday. Benefits Medical, Vision, and Dental insurance Short‑Term Disability, Life Insurance, Critical Illness coverage Generous PTO package Flexible hours with 9AM–4PM core Highly competitive salary and generous bonus $1,000 continuous education reimbursement and training Full‑time virtual scribe assistance ($1,368/month value) Earn minimum 33 CEUs at no cost Access to CRC's Physiatry certification and CEU credits 401(k) plan with 2–3% annual contribution 100% malpractice insurance coverage Laptop provision Responsibilities Provide specialized PM&R/Physiatry services as a rehab consultant Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team Collaborate with PCP teams to manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries Work with PT, OT, and SLP teams to improve patient function and quality of life Follow therapy patients to track progress Assess and manage barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and more Attend multidisciplinary meetings and bridge gap between therapy and internal medicine Engage in culture‑building opportunities and ongoing education, including monthly didactic lectures, virtual regional‑team meetings, and annual regional dinners Qualifications Nurse Practitioner / APRN / Physician Assistant California license Ability to work in the U.S. Experience in post‑acute space (preferred) Great attitude and desire to deliver best patient care 40 hours/week Ideal Candidate Profile Passion for post‑acute healthcare and serving geriatric population SNF or post‑acute experience (preferred) Confidence to function autonomously Desire for continuous improvement and ability to embrace change and growth Strong interpersonal skills, empathy and compassion Desire to invest in company culture Licensure / Certification Must hold a Nurse Practitioner / APRN / PA license; California licensed; U.S. work eligibility required. #J-18808-Ljbffr Comprehensive Rehab Consultant
