As a Nurse Practitioner specializing in home health services, you will visit patients for risk assessments in various communities within Madison County, such as Edwardsville, Alton, and Granite City. Each assessment typically takes about 45 minutes and could range from comprehensive annual evaluations to post-discharge visits.

Schedule/Hours:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform home health risk assessments for Medicare Advantage plan members
• Assess medical history, current conditions, and medication regimens
• Offer education regarding patients medical conditions and medications
• Identify gaps in care and recommend steps for improvement
• No treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Illinois
• MSN degree from an accredited nursing program
• Current CPR (BLS) certification
• Board certification through AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
