Nurse Practitioner Palliative Care

Nurse Practitioner - Palliative Care Salary: $100,000 - $110,000 per year depending on experience. Licensed independent practitioner responsible for managing health problems and coordinating health care for palliative care patients in accordance with state and federal regulations and nursing standards of care. Clinical management is conducted in collaboration with hospice and home health team members. Responsibilities Perform age-appropriate history and physical for complex, acute, critical, and chronically ill patients. Order and interpret diagnostic and therapeutic tests relative to patient’s age‑specific needs. Prescribe pharmacologic and non‑pharmacologic treatment modalities. Implement interventions to support patients with chronic conditions to decrease repeat hospitalizations. Assist with provision of palliative care in accordance with facility, state, and federal regulations. Monitor effectiveness of interventions. Facilitate patient transition within and between health care settings, such as admissions, transfers, and discharges. Collaborate with primary care physicians/NPs by making appropriate referrals, follow‑up calls, and coordinating care. Facilitate patient and family decision making by providing education on palliative care, end‑of‑life care, advance directives, and chronic care management. Maintain accurate, legible bedside documentation in medical records using appropriate billing codes. Qualifications Graduate of an accredited school of nursing. Graduate of an accredited Nurse Practitioner Program. Current Advanced Practice Registered Nurse (APRN) licensure. Certification in Hospice or Palliative Care within one year of starting the position. Strong computer skills, including Word, Excel, and PowerPoint. Minimum five years of clinical hospice and palliative care experience; two years preferred in the Nurse Practitioner role. Benefits Competitive salary. Bonus eligibility.
