Job Summary: The Nurse Care Coordinator provides comprehensive nursing care, education, and support to patients, ensuring coordinated, patient-centered services across various clinical settings. The role emphasizes clinical judgment, effective communication, and collaboration to promote optimal patient outcomes.

Responsibilities:

• 
  Perform detailed patient assessments, developing and updating individualized care plans based on clinical findings.
  
  
• 
  Administer medications and treatments accurately, following provider orders and safety protocols.
  
  
• 
  Recognize clinical changes promptly and implement appropriate interventions or escalate when necessary.
  
  
• 
  Collaborate with interdisciplinary teams to coordinate care, facilitate referrals, and ensure continuity of services.
  
  
• 
  Document assessments, interventions, and patient responses accurately and in a timely manner within electronic health records.
  
  
• 
  Educate patients and families on care plans, medication management, and discharge instructions to promote self-care and health literacy.
  
  

Qualifications:

• 
  Hold an associates degree in Nursing from an accredited program.
  
  
• 
  Possess a current, unrestricted Registered Nurse license in the State of Alaska.
  
  
• 
  Maintain Basic Life Support (BLS) certification.
  
  
• 
  Demonstrate strong clinical judgment, organization, and communication skills.
  
  
• 
  Be able to work independently in a fast-paced environment and collaborate effectively within teams.
  
  
• 
  Complete clinical competency requirements within 3 months and every 3 years, including high-risk competencies annually.
