Hospice RN Case Manager — 1:1 Home Care Leader

St. Croix Hospice in West Des Moines, IA is seeking a Registered Nurse Case Manager who will provide compassionate 1:1 support for patients and families during the end-of-life journey. The nurse will work with an interdisciplinary team to create and adjust individualized care plans, ensuring comfort and dignity. This full-time position includes a competitive benefits package, including health insurance, 401(k) with company match, and tuition reimbursement.
