Salary: $90,000 – $95,000 annually (based on experience)
Position Overview
A leading home health organization is seeking an experienced Patient Care Manager (RN) to provide clinical leadership and oversight for a team of healthcare professionals serving patients throughout the Temple area. This role is responsible for ensuring high-quality patient care, supporting clinical staff, maintaining regulatory compliance, and driving positive patient outcomes.
The Patient Care Manager serves as a key leader within the organization, fostering a collaborative environment focused on excellence, safety, and compassionate care.
Key Responsibilities
Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements.
Coordinate, plan, and monitor patient care services to achieve optimal clinical outcomes.
Supervise and support clinical staff, ensuring consistent and appropriate delivery of home health services.
Maintain effective communication with patients, caregivers, referral partners, field clinicians, and office personnel.
Support quality improvement initiatives and departmental performance goals.
Promote professionalism, accountability, and service excellence throughout the organization.
Ensure compliance with Medicare guidelines and applicable federal, state, and local regulations.
Maintain a safe work environment and reinforce safety protocols.
Mentor and develop staff through coaching, education, and ongoing professional development opportunities.
Qualifications
Current Registered Nurse (RN) license in the state of Texas.
Previous home health experience required.
Two or more years of management or supervisory experience preferred.
Strong knowledge of Medicare Conditions of Participation and home health regulations.
Excellent leadership, communication, organizational, and problem-solving skills.
Ability to effectively manage multiple priorities in a fast-paced healthcare environment.
