Registered Nurse LTC/SNF

Job Summary:

Responsibilities:

• 
  Assess patients' health status and develop individualized care plans.
  
  
• 
  Administer medications, treatments, and physician orders accurately.
  
  
• 
  Collaborate with nursing aides, therapists, and physicians to ensure holistic patient care.
  
  
• 
  Monitor patient progress and update care plans as needed.
  
  
• 
  Educate patients and families on recovery processes and health management.
  
  
• 
  Maintain accurate documentation of care activities and patient responses.
  
  

Qualifications:

• 
  Valid RN license in the state.
  
  
• 
  Experience in geriatric, post-op, or rehabilitative nursing preferred.
  
  
• 
  Knowledge of EPIC systems and WV Cares databases.
  
  
• 
  Ability to work in a fast-paced healthcare environment.
  
  
• 
  Strong communication and teamwork skills.
  
  
• 
  Basic life support (BLS) certification required.
