You’ll work independently in the field – but never alone. Our Clinical Supervisors and operational staff are readily available to support you. Responsibilities Deliver skilled nursing care in accordance with the established plan of care. Monitor and assess patient conditions during home visits or shifts. Communicate changes in patient status to the Clinical Supervisor. Provide patient and caregiver education regarding medications, treatments, and disease management. Accurately document patient care and clinical observations. Coordinate care with other healthcare team members as appropriate. Maintain compliance with agency and state guidelines. Schedule Options Day, evening, overnight shifts available. Weekday and weekend opportunities. Full‑Time, Part‑Time, PRN, Per Diem. Choose your own hours based on availability. We work with you to build a schedule that supports your lifestyle. Compensation $36.00 - $41.00 per hour. Pay rate based on experience, case complexity, and assignment type.
