It is important to us that service standards exceed the industry average. Position Summary We are seeking a Registered Nurse (RN) to work within the St. Louis region. The RN will administer skilled nursing care to patients in their homes in accordance with physician orders and the plan of care under the direction and supervision of the Director of Clinical Services. Responsibilities Performs the initial evaluation visit and regularly re-evaluates patient’s nursing needs. Initiates the plan of care and necessary revisions. Participates in basic patient care, depending on diagnoses and required precautions. Prepares clinical and progress notes for each patient visit and summarizes care conferences in a timely manner as per Agency policy. Coordinates a variety of services as needed. Counsels the patient and family/significant others to meet nursing and related needs. Participates, trains, and prepares for in-service programs as assigned. Processes orders and notifies the physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries. Refers to physical therapists, speech pathologists, occupational therapists, and medical social workers. Supervises and teaches other nursing personnel. Conducts patient care conferences for patients assigned to his or her care. Is on-call as assigned. Required Qualifications Access screens, triage and perform treatment ordered for patients. Perform medical treatment as ordered by physician. Prepare and dispense prescribed medications. Assist with examinations, procedures and other duties. Oversee and coordinate daily patient care. Assist patients as needed with walking, transfer, specimen collection, exam preparation, etc. Complete EMR documentation and/or paper-based documentation. Perform other duties as assigned such as new models of care and new types of procedures. Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing. Two (2) years of experience in nursing preferred.
