LPN Nurse Care Manager

Description: Job Overview Seeking a Licensed Practical Nurse (LPN) to support children and families in a community-based Preventive Services Program. This role focuses on health care coordination, family education, home visits, and connecting families with medical and community resources. Responsibilities Coordinate healthcare services for children and families Conduct home visits and provide health education and support Review medical reports and coordinate follow-up care Assist families in accessing healthcare and community resources Collaborate with social workers, physicians, schools, and community providers Monitor children's healthcare needs and maintain documentation Accompany families to appointments as needed Support health monitoring and care planning for high-risk families Requirements Current New York State LPN License Experience in community-based nursing, home care, care coordination, behavioral health, or pediatrics preferred Strong communication and organizational skills Ability to travel locally for home visits and appointments Additional Information This position includes field visits and direct interaction with children and families in the community.
