Care Manager (RN) - Outpatient Oncology

Scope of Position / Position Summary The Patient Care Resource Manager (PCRM) is a nurse case manager who facilitates patients through the health care system, from pre-admission through post-discharge coordination of care, resources, and services utilizing the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions, throughput, and care quality and efficiency, and ensures efficient resource utilization. The PCRM identifies and removes barriers to the progression of the plan of care for each patient. Minimum Education Required Bachelor’s Level Degree or equivalent combination of education and experience. Required Qualifications Bachelor’s degree, Master’s preferred. RN licensure required. Five years recent clinical experience applicable to the population preferred. Knowledge of clinical practice guidelines, computer databases, and payer protocols desirable. Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post‑offer process.
