Medical Social Worker (MSW) – PRN

YOU MUST MEET ALL OF THE FOLLOWING REQUIREMENTS TO BE CONSIDERED:

• Master’s Degree in Social Work (MSW)
• Active, FULL and unrestricted social work license
• LIMITED, TEMPORARY, ASSOCIATE, PROVISIONAL, OR RESTRICTED LICENSES WILL NOT BE ACCEPTED
• Valid driver’s license
• Reliable personal vehicle
• Current auto insurance with your name listed on the policy

Applicants must currently hold a  full, active, unrestricted social work license .

If your license is limited, associate-level, temporary, provisional, or otherwise restricted, you will not qualify for this position.

Guardian Angel Home Care is seeking an experienced  Medical Social Worker (MSW) to join our San Diego home health team on a  PRN / Per Diem basis .

This position is ideal for an experienced social worker looking for  flexibility, supplemental income, and strong compensation without a full-time commitment .

• Complete psychosocial assessments
• Evaluate patient and family needs
• Provide counseling and emotional support

• Assist with financial, transportation, housing, and long-term care resources
• Support patients and families with adjustment to illness or disability
• Participate in interdisciplinary care planning
• Coordinate with nurses, therapists, physicians, and other clinical staff
• Document visits and interventions accurately and timely
• Travel to patients’ homes throughout the assigned service area

• Master’s Degree in Social Work (MSW) — REQUIRED
• Full, active, unrestricted social work license — REQUIRED
• Limited licenses will not be accepted
• Home health, hospice, hospital, rehabilitation, or community-based experience preferred
• Strong knowledge of community resources
• Excellent communication and documentation skills
• Ability to work independently in patients’ homes
• Reliable, compassionate, and professional

This is a  field-based PRN home health position .

You must have:

• Valid driver’s license
• Reliable personal vehicle
• Current auto insurance
• Your name listed on the insurance policy

• High per-visit compensation
• Hourly compensation may be offered depending on assignment
• PRN / Per Diem scheduling
• Flexible availability
• Additional patient assignments may be available based on census and coverage needs

Patients’ homes throughout  San Diego County, California

Travel between patient locations is required.

We are currently seeking an experienced  PRN Medical Social Worker for our San Diego home health team.

✅ I have an  MSW
✅ I hold a  FULL, ACTIVE, UNRESTRICTED social work license
✅ My license is  NOT limited, temporary, associate, provisional, or restricted
✅ I have a valid driver’s license
✅ I have a reliable vehicle
✅ I have current auto insurance with my name listed on the policy
