Responsibilities Provide personal care and housekeeping assistance to clients in their homes, following the established plan of care under the direction of the client care coordinator. Perform personal hygiene and grooming care for clients. Provide meals to meet clients nutritional needs. Report changes in clients condition to the client care coordinator or supervisor. Carry out routine housekeeping and home-management duties. Record each client’s care on the care plan and submit the plan in a timely manner. Respond sensitively to the multiple, complex needs of a terminal client, including providing end-of-life care. Assist with routine therapy exercises. Participate in case conferences as requested. Qualifications Education: High school diploma or equivalent (preferred). Licensure/Certification: CNA certificate approved by the Connecticut Department of Public Health and Addiction Services (required). Experience: Six months experience as a Home Care Aide/CNA in a home-care agency (preferred).
