Requirements: Current state license as a Registered Nurse; Homecare Homebase Software Experience - Required; OASIS Experience - Required; Proof of current CPR certification - Required; 1-2 years experience as a Registered Nurse in a clinical care setting - Required; 2+ years Home health experience - Required.

Responsibilities:

• Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
• Educate patients and families regarding care, medications, diet, and home safety, promoting self-care practices.
• Coordinate referrals to appropriate agencies when needed.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
