Family Planning Nurse Practitioner

**Job Summary:**
The Community Health Center of Buffalo, a Federally Qualified Health Center (FQHC), is actively seeking an experienced Family Planning Nurse Practitioner to provide comprehensive outpatient care to patients at our modern facility. **Minimum Qualifications:**
- Licensed NYS, board-certified, Registered Professional Nurse Practitioner.
- Experience as a Family Planning Provider, including Long-Acting Reversible Contraceptives.
- Computer literacy with proficiency in MS Word, Excel, and Outlook. Experience with eClinical Works is preferred.


**Qualifications:**
- Active Nurse Practitioner license
- Certification in Family/Adult Health Nurse Practitioner
- In-depth knowledge of family planning and contraceptive methods
- Excellent communication and counseling skills
- Strong commitment to providing compassionate and nonjudgmental care


**Responsibilities:**
- Conduct comprehensive health assessments for individuals seeking family planning services
- Develop personalized family planning strategies based on individual patient needs
- Educate patients on various contraceptive options, including benefits and potential side effects
- Prescribe medications and contraceptives as needed
- Collaborate with interdisciplinary healthcare team members to ensure holistic patient care
- Maintain accurate and up-to-date medical records
- Adhere to all relevant laws and regulations governing family planning services
