IDDT Specialist Case Manager

Responsibilities The essential function is to work in partnership with Dual Diagnosed people receiving services based upon specific objectives identified in the treatment plan. The approach is strengths based using the IDDT model. Staff training in the IDDT model will be conducted. The essential function of the case manager is to work in partnership with people receiving services toward individual goal attainment based upon specific objectives identified in the treatment plan. The approach is strengths based, and most of the service is provided in the community. Case managers must be able to transport people served, have a reliable vehicle, a valid Kansas driver’s license and necessary insurance coverage. Must be able to pass a KBI check and must not be listed on any registries. Each case manager is responsible for an assigned caseload.
