About the Role

Responsibilities
- Evaluate patients and develop individualized therapy plans
- Provide skilled physical therapy services in a SNF/LTC setting
- Support mobility, strength, balance, and functional improvement
- Document treatment progress and patient response accurately
- Collaborate with the care team to support patient outcomes
- Maintain safe therapy practices and follow facility standards
- Active Texas Physical Therapist license required
- Minimum 1 year of Physical Therapist experience required
- Skilled Nursing Facility or Long-Term Care experience preferred
- Strong clinical assessment and patient care skills
- Ability to work independently and as part of a care team
